Detailed registration and renewal intake
Intake forms also collect facility identity and address, D-U-N-S/UFI, FDA registration number for renewal, ownership, parent company, facility and emergency contacts, product categories and activities, seasonal dates, U.S. agent details, changes, submitter authority, and consent to review. Do not submit FDA passwords or PINs. Only authorized staff can access the dashboard.

When automated email is configured, Resend processes notifications to support@usagent.com, including the inquiry reference, service and language; full intake information remains in the dashboard. Replies are handled through the support mailbox. Website hosting and database providers process information to operate this website. Information may also be disclosed where required by applicable law or as necessary to carry out a separately agreed service. This website does not sell inquiry information or enroll you in a marketing newsletter.
